Mirabillon is the first beta adrenergic receptor agonist drug used to treat bladder overactivity, and its successful launch fills the gap in the use of beta adrenergic receptor agonists for the treatment of bladder overactivity.
Mirabillon mainly acts on β 3 receptors to relax the bladder detrusor muscle and increase its stability. Three subtypes of β receptors (β 1, B2, and β 3) were found in human detrusor muscle cells and urethral epithelial cells. The mRNA of β 3 receptors is mainly expressed in human detrusor smooth muscle cells, accounting for 97% of the β receptor mRNA in bladder tissue. The expression of β receptor mRNA and its signaling pathway function suggest that β 3 receptors play a pivotal role in both normal and diseased bladder.

Application&Function

Mirabillon is prone to interact with clinically relevant drugs such as cytochrome CYP2D6 substrates and CYP3A4 inhibitors. Research has shown that Mirabilon increases plasma exposure to demethylamine (drugs with CYP2D6 substrates), reversibly prolonging drug half-life. Other studies have shown that different doses of ketoconazole (a strong CYP3A4 inhibitor) can inhibit the clearance rate of Mirabillon in healthy adults, while repeated doses of rifampicin (inducing CYP3A4 metabolism) can lead to moderate reductions in Mirabillon plasma concentrations.
