Glimepiride is a third-generation sulfonylureas long-acting anti-diabetes drug. The main mechanism of its hypoglycemic effect is to stimulate the secretion by beta cells of the pancreas and partially increase the sensitivity of surrounding tissues. Its mechanism is through and sulfonyl urea receptors on the surface of the pancreatic beta cells (relative molecular mass of 6.5 x 104 protein), the receptor and ATP sensitive K + (KATP) channel phase coupling, pushing the KATP channel close, cause cell membrane depolarization, makes the open voltage dependent calcium channel, inhibit the synthesis of liver glucose.

Function
Glimepiride is a third-generation sulfonylurea long-acting antidiabetic drug. Its mechanism of action is to bind to the sulfonylurea receptor on the surface of pancreatic β-cells, which is coupled to ATP-sensitive K+ (KATP) channels. Promoting the closure of KATP channels, causing the depolarization of the cell membrane, the opening of voltage-dependent calcium channels, the influx of Ca2+, the release of insuline, and the inhibition of hepatic glucose synthesis. In addition, glimepiride can also increase cardiac glucose uptake through an insuline-independent pathway, which may be caused by the increased expression of two proteins, glucose transporter 1 and 4. Because glimepiride has a weak effect on cardiovascular KATP channels, adverse cardiovascular reactions are also rare.
Application
1. It is suitable for type 2 diabetes mellitus whose blood sugar cannot be adequately controlled by diet, exercise therapy and weight loss.
2. Glimepiride dispersible tablets are not suitable for the treatment of type 1 diabetes (eg, the treatment of diabetic patients with a history of ketoacidosis), diabetic ketoacidosis, or the treatment of pre-diabetic coma or coma.
