Domperidone is a commonly used prokinetic and antiemetic drug, commonly known as metoclopramide. This product is a potent peripheral dopamine receptor antagonist, capable of blocking DA1 and DA2 receptors before and after synapses, and antagonizing dopamine's inhibitory effect on cholinergic neurons in the intestinal muscle plexus postsynaptic, leading to gastric contraction. It can promote the normal recovery of peristalsis and tension in the upper gastrointestinal tract, facilitate gastric emptying, increase the movement of the gastric antrum and duodenum, coordinate the contraction of the pylorus, and also enhance esophageal peristalsis and tension of the lower esophageal sphincter. Due to its poor permeability to the blood-brain barrier and almost no antagonistic effect on dopamine receptors in the brain, it can rule out side effects on the mental and central nervous system. After oral administration, the drug is rapidly absorbed and reaches its peak blood concentration within 15-30 minutes. Except for the central nervous system, it is widely distributed in other parts of the body. Due to the presence of first pass effects on liver metabolism and intestinal wall metabolism, oral bioavailability is relatively low.

Application&Function

Domperidone acts on peripheral dopamine receptors, promoting gastrointestinal peristalsis, improving the coordination of the gastric antrum and duodenum, increasing the tension of the lower esophageal sphincter, and improving gastroparesis. Erythromycin can also enhance gastric motility, stimulate gastrointestinal peristalsis, and has no effect on normal individuals. Its dosage is small and safe. The combination of the two has synergistic effect and can be used for the treatment of diabetes patients with gastroparesis.
1. Symptoms of indigestion caused by delayed gastric emptying, gastroesophageal reflux, chronic gastritis, esophagitis, including nausea, vomiting, belching, upper abdominal distension, abdominal pain, bloating, and burning sensation in the mouth and stomach due to reflux.
2. Parkinson's disease and nausea and vomiting caused by various reasons, such as functional, infectious, dietary, radiation therapy or drug therapy, as well as treatment with dopamine receptor agonists (such as levodopa, bromothalonil).
3. Suitable for migraine, dysmenorrhea, traumatic brain injury and intracranial lesions, radiotherapy, as well as nausea and vomiting caused by levodopa, nonsteroidal anti-inflammatory drugs, etc.
4. It is also effective for nausea and vomiting caused by various organic or functional gastrointestinal disorders in elderly people. 5. Domperidone is ineffective against postoperative vomiting or vomiting caused by anesthesia or chemotherapy.
